Patient Forms
Upon entering our office, we will collect the patient intake questionnaires that you have completed. We appreciate you thoroughly completing these forms, as this helps Dr. Sefcik in adequately dealing with your needs. Please be ready to provide your insurance card, if applicable, so we may make a copy. If you have any outside medical records and/or lab reports that you would like Dr. Sefcik to review please bring these with you, or preferably, have them sent to our office a few days before your initial appointment.
Patient History & Review of Systems
After reviewing your intake forms, Dr. Sefcik will meet with you to address your major health concerns. To make the most of this time, please prioritize your major health concerns, as well as your specific questions regarding these concerns. A detailed medical and family history is a key component in finding the underlying source of dysfunction in your health.
Examination
After your history, Dr. Sefcik will provide a very thorough examination. As a board certified chiropractic neurologist, as well as North Dakota's only certified clinical nutritionist, Dr. Sefcik utilizes these areas of expertise to guide his examination. Because Dr. Sefcik performs a complete functional neurologic evaluation (i.e. muscle strength, reflexes, sensation, cranial nerve examination, cerebellar function and balance), musculoskeletal evaluation (i.e. posture, gait, muscle tone), as well as other tests, comfortable clothing is recommended. Patient gowns are also available.
X-Ray Studies
Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.
Report of Findings
Once the information is collected and examinations are performed, Dr. Sefcik will give you a detailed report of all findings and answer any questions including:
- What is wrong with me?
- Can you help me?
- How long will it take?
- What will my treatment cost?
After reviewing your health history, pertinent lab information and imaging studies, and your personal health goals, Dr. Sefcik will discuss recommendations and notify you if your condition requires care with other providers. Dr. Sefcik will provide the best treatment and wellness program for your needs.
We are often asked this question, "Can you help me?" We treat you as an individual, not as a disease. Dr. Sefcik acquires as much information as possible to fully assist him in determining your major health blocks. We know that making seemingly small changes in your life can have profound positive effects on your health. For example, doesn't it make sense that if you would starting eating better, moving better, drinking pure and refreshing water that you would begin to notice improvement? Our goal is to build a solid foundation and provide an environment in which your body can heal. We feel that everyone can benefit from life enhancement changes.
The duration of care depends on your current health. If you are healthy and active and face a minor health setback it won't take very long to get you back on the right track. However, if you are facing chronic, lifelong, complex health conditions your duration of care will require a more active approach and a personal commitment for making positive lifestyle choices. The choice is always yours.
Dr. Sefcik likes to rephrase the fourth question as, "how much is your health worth?" The cost of care is dependent on your individual treatment recommendations. These will be discussed with you during your report of findings. Our office does accept most health insurance policies. Payment plans are also available upon request.
Treatment
We provide a variety of treatment options and modalities to individualize your care. Dr. Sefcik will choose those that he feels are most beneficial to you.
Wellness Program
Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
